I have put together a list of major league and minor league pitchers with the approximate, if not exact, surgery date and, when applicable, the date that they returned to official game action (does not include rehab assignments). Players are listed in order of the date of surgery. There is one list for players who were on a 40-man roster at the time of surgery and another for minor leaguers who were not. Some of the minor leaguer injuries fly under the radar so please let me know if I'm missing anyone. The player is listed on the organization they were with at the time of the surgery.

Is there a database that you are aware of that lists which pitchers have had tommy John surgery and how long they were out for?
Don't think so. I will be keeping one in the near future for MLBDepthCharts Magazine but I'll only be going back 1-2 years (as far back as I can find info). Let me know if you do come across one.
I compiled a more complete list of Tommy John surgeries that I hope to maintain over time, as well. There are 488 confirmed TJ surgeries on the list as of today.

I saw it. Great job! It really seemed like there were more surgeries the past few years but it's been pretty steady for the past decade. I guess I've just been paying closer attention as I maintain this site.
